  SYMPTOMS - what the patient reports (6)
    - Pain is usually mild, felt as dryness, itching, soreness, irritation, or a burning feeling
      [itching]
    - Discomfort stays localized to the lesion and gets worse on opening the mouth
    - Severe disease can impair eating, though it is rarely the main driver of malnutrition
    - Ask about diarrhea, blood in the stool, or abdominal pain pointing to Crohn disease

    - Dry eyes or dry mouth can point toward Sjogren syndrome  [dry mouth]
    - History should cover prior dental work and denture wear
  SIGNS - what you find (8)
    - Red, swollen, tender triangular patches at the corners of the mouth
    - Mild disease shows pink erythema with lips otherwise normal or just chapped  [redness]
    - Moisture causes superficial maceration and erosion with gray-white patches rimmed by redness
      [redness]
    - Moderate lesions can look papular, eczematous, and fissured; older lesions turn bluish-white
      with scale  [papules]
    - Severe fissuring may bleed, and chronic lesions can weep, crust, or form granulation tissue
    - Honey-colored discharge or pustules point to bacterial superinfection  [pustules]
    - Lesions are usually bilateral and symmetric; a one-sided lesion without a clear cause needs
      further work-up
    - Fissures extending past the lip border track along the marionette lines
  TESTS (7)
    - Diagnosis rests on clinical exam; lab testing is kept for cases not responding to first-line
      treatment
    - Candida testing or bacterial culture is often done at diagnosis since infection is the leading
      cause
    - No response to antifungal or antibiotic treatment after 2 to 3 weeks prompts a wider work-up:
      blood count with MCV, iron studies with ferritin, folate, B2, B6, B12, and fasting glucose
    - PAS-stained scrapings under the microscope show hyphae for infection or yeast for colonization
    - Oral candidiasis on exam should prompt HIV testing and a diabetes work-up
    - Patch testing helps separate allergic from irritant contact cheilitis
    - Biopsy is used when malignancy is suspected

    - Erosive oral lichen planus or other lichenoid oral lesions
    - Impetigo or atopic dermatitis can mimic the corner-of-mouth lesions

    - Allergic or irritant contact cheilitis from lip products or habits
    - Actinic cheilitis, especially when the corners go unprotected by sun-blocking lip balm
    - Cheilitis glandularis
    - Cheilitis granulomatosa or exfoliative cheilitis

1. MICONAZOLE                                             [1st line]
   Adult    Apply 2% oral gel thin layer to labial commissures 4 times daily after meals, continuing
            for at least a week after symptoms disappear x 10-14 days, continuing for a week after
            the corners have healed
   Peds     Age restriction: minimum 4 months
            (Child >4 months: apply a thin layer of 2% oral gel to labial commissures 4 times daily
            after meals)
   Choice   Alternatives. The entry's rationale names miconazole first-line: it covers both the
            Candida that usually causes angular cheilitis and the staphylococci that often accompany
            it.

   Why      First-line topical antifungal with potent inherent Gram-positive antibacterial activity
            against staphylococci
   Caution  Miconazole oral gel interacts significantly with oral anticoagulants (Warfarin) causing
            severe bleeding risk; avoid co-administration.
            Investigate underlying predisposing factors: ill-fitting dentures, iron deficiency
            anemia, B12/folate deficiency, diabetes.
            The oral gel is barred under 4 months, and at any age if the swallowing reflex is not
            fully developed. It has choked infants. Never put it at the back of the throat: divide
            the dose, smear it on with a clean finger, and watch the child.
            Continue a week past the point the corners look healed. Stopping the day they clear is
            why it returns.
            Avoid in liver dysfunction.
