   Peds     2 to under 6 years: 5 mg once daily
            6 years and over: 10 mg once daily

            Alternative weight-based dosing for children under 12 years: 30 kg or less, 5 mg once
            daily; over 30 kg, 10 mg once daily. Under 2 years: safety and efficacy not

   Why      A drug eruption is an itchy, often urticarial or morbilliform rash; a standard non-
            sedating oral antihistamine controls the itch while the causative drug is withdrawn, the
            same symptomatic role it plays in urticaria.
   Caution  Excreted in breast milk; the formulary does not recommend use in breastfeeding women.
            Renal impairment with creatinine clearance under 30 mL/min: 10 mg every 48 hours
            Signs of a severe cutaneous adverse reaction - mucosal involvement, blistering, skin
            sloughing, or systemic symptoms - need emergency referral for possible Stevens-Johnson
            syndrome or DRESS.
            Identify and stop the suspected causative drug - this is the definitive treatment; the
            antihistamine only controls itch while the eruption resolves.
            Urgent same-day referral if there is mucosal (mouth, eye, genital) involvement,
            blistering, skin sloughing, or systemic symptoms - these suggest Stevens-Johnson
            syndrome or DRESS, not a simple exanthematous drug eruption.
            Document the reaction and the culprit drug clearly in the patient's record/allergy list
            to prevent re-exposure.
            Loratadine is non-sedating; use caution in severe hepatic impairment.
