Symptoms — what the patient reports (6)

  • Lower limb claudication - pain or cramping on walking that eases with rest - suggests peripheral artery disease from dyslipidemia [claudication · muscle cramps]
  • Angina - chest pain brought on by exertion or emotional stress, radiating to the neck, jaw, shoulders, or back [chest pain]
  • Sudden weakness, slurred speech, or visual disturbance can signal a TIA or stroke from atherosclerosis [slurred speech]
  • Blurred vision can occur with lipemia retinalis in severe hypertriglyceridemia [blurred vision]
  • It usually produces nothing the patient can feel, and turns up on a blood test
  • The primary form is inherited, from mutations that affect how lipids are handled; the secondary form is acquired, from the way a person lives or from another illness

Signs — what you find (3)

  • Yellowish fat deposits (xanthomas) on the eyelids, palms, or tendons point to high cholesterol or triglycerides
  • A grey or white ring at the corneal margin - arcus senilis - reflects high blood cholesterol
  • A milky appearance of the retinal vessels on fundoscopy reflects very high triglyceride levels

Tests (6)

  • A fasting lipid panel measuring total cholesterol, LDL, HDL, and triglycerides is the primary evaluation tool
  • LDL cholesterol under 100 mg/dL is optimal, rising through borderline-high and high categories
  • Triglycerides are normal below 150 mg/dL and borderline-high from 150 to 199 mg/dL
  • HDL should be over 40 mg/dL in a man and over 50 in a woman
  • Total cholesterol should sit under 200 mg/dL
  • Total cholesterol is LDL plus HDL plus half the triglyceride level, and a high one flags the risk of heart disease and stroke

Choice

Alternatives. Atorvastatin carries the larger outcome evidence base and is the cheaper to start (12-128 EGP against 26-368).

Why

HMG-CoA reductase inhibitor (statin) that lowers LDL cholesterol by blocking hepatic cholesterol synthesis, used to reduce cardiovascular risk in dyslipidemia.

Cautions
  • CONTRAINDICATED in pregnancy and breastfeeding.
  • Monitor LFTs before starting and at 12 months.
  • Muscle pain requires creatine kinase check.
  • Caution in heavy alcohol use or hepatic disease.

Why

Why

Fibrate that activates PPAR-alpha to lower triglycerides, used for severe hypertriglyceridemia that is not adequately addressed by a statin, since statins primarily target LDL rather than triglycerides.

Cautions
  • Monitor renal function and LFTs.
  • Fenofibrate dosing depends on the formulation - the formulary gives 200 mg for the capsule and directs you to the product leaflet. The 160 mg here is the tablet strength.
  • There is a risk of cholelithiasis and gallbladder disease.
  • There is an increased risk of myopathy when combined with statins.
