   Adult    This is an obstetric history record indicating prior assisted vaginal birth by forceps.
            In primary care, evaluate the patient for long-term maternal pelvic floor complications,
            including urinary incontinence, fecal incontinence, pelvic organ prolapse, and perineal
            pain or dyspareunia related to prior third- or fourth-degree tears. In subsequent
            pregnancies, review the indication for prior operative delivery, confirm maternal
            perineal healing, and coordinate antenatal care with obstetrics. - Refer
