Red flags (3)

  • A strangulated hernia is an irreducible hernia whose blood supply is compromised - ischaemia progressing to necrosis, and a surgical emergency
  • An incarcerated hernia is irreducible, usually because of a narrow neck, but the contents are still viable and not obstructed
  • Elective repair is recommended to avoid incarceration and strangulation, but a reducible hernia can be safely observed in an elderly, sedentary patient or one at high surgical risk

Symptoms — what the patient reports (2)

  • Typically asymptomatic until a groin lump or swelling is noticed [hernia]
  • Some report groin pain on straining or heavy lifting; pain and discomfort mostly go with larger hernias [groin pain]

Signs — what you find (5)

  • History and clinical examination make the diagnosis; no imaging is needed unless there are extenuating circumstances
  • Examine both groins supine, then standing, then with coughing and straining, to find small reducible hernias [cough]
  • Palpate the external ring by invaginating the scrotum with the index finger to a point lateral and superior to the pubic tubercle
  • Coughing or straining during that examination is what makes the protruding tissue palpable [cough]
  • A larger hernia may need manual compression, or lying supine with compression, before it will reduce [hernia]

  • Inguinal hernias are about 75% of all abdominal wall hernias
  • An indirect hernia passes through the deep inguinal ring lateral to the inferior epigastric vessels; a direct hernia passes through the transversalis fascia in Hesselbach triangle, medial to those vessels
  • Reducible means the contents go back spontaneously or with manual pressure; irreducible means they do not
  • CT or ultrasound help if bowel obstruction is possible, but are not needed before surgery

NO DRUG THERAPY REQUIRED (SURGICAL REFERRAL)

1st line
Adult dose and duration

Elective general surgery referral for surgical evaluation and mesh hernioplasty. Watchful waiting is acceptable only in asymptomatic or minimally symptomatic adult men. - Elective referral

Paediatric dose

Pediatric inguinal hernias (patent processus vaginalis) carry a high risk of incarceration; urgent pediatric surgical referral is required for early repair.

Why

Inguinal hernia is an anatomical structural defect requiring surgical repair; no pharmacological treatment repairs the hernia defect.

Cautions
  • EMERGENCY WARNING: Irreducible, painful hernia with severe acute pain, vomiting, bowel obstruction, or overlying skin discoloration indicates incarceration or strangulation — refer immediately for emergency surgery.
  • Trusses and abdominal binders are NOT recommended for routine management as they do not prevent incarceration and may cause testicular atrophy or local skin damage.
  • Surgical repair is recommended for all symptomatic inguinal hernias and all female groin hernias (to exclude femoral hernia).

500-1000 mg three or four times a day PRN for mild localized groin discomfort while awaiting elective surgical consultation (maximum 4000 mg/24 hours) - PRN until surgical evaluation

Why

Simple analgesic added for mild groin discomfort while awaiting elective surgical assessment of an inguinal hernia; it provides symptomatic relief only and does not reduce the hernia or remove the need for surgical referral, especially urgently if the hernia becomes irreducible or pain worsens suddenly.

Cautions
  • Analgesia provides temporary mild symptomatic relief only and does not eliminate the need for surgical assessment.
  • Instruct patient to seek emergency care immediately if hernia becomes non-reducible or pain suddenly worsens.
