SALBUTAMOL

1st line

Formoral.inhalation

Adult dose and duration

Oral inhalation, one inhalation (100 mcg) or two inhalations up to 4 times a day for persistent symptoms. The maximum daily dose should not exceed 8 inhalations and should usually not be repeated more often than every 4 hours. - As directed, for symptomatic episodes

Paediatric dose

(Children under 12 years: one inhalation (100 mcg) or two inhalations up to 4 times daily, maximum daily dose 800 mcg and no more than 6 times in 24 hours; over 12 years, dose as per adults.)

Why

Salbutamol is the formulary-indicated bronchodilator for bronchospasm of any cause and is the appropriate first-line, as-needed treatment for airway hyperreactivity presenting with cough, wheeze, or chest tightness.

Cautions
  • Inhaled, not swallowed: the dose is the metered-dose inhaler's 100 mcg per actuation.
  • Severe breathlessness, inability to speak in full sentences, or a silent chest is a medical emergency - needs immediate high-dose salbutamol, oxygen, and urgent transfer, not routine dosing.
  • Tremor, tachycardia, and palpitations are common; caution in cardiac disease and arrhythmia.
  • Frequent as-needed use (more than a few times a week) suggests the need for regular preventer therapy rather than reliever alone.

Adult dose and duration

Mild asthma: starting dose is 100 micrograms twice daily. - As directed, reviewed periodically and titrated down once controlled

Paediatric dose

(Oral inhalation is only specified for children over 16 years in this monograph: starting dose 50-100 micrograms twice daily, maximum 200 mcg twice daily if not sufficiently controlled; dosing for younger children with reactive airways is specialist-set and is not reproduced here.)

Why

Fluticasone oral inhalation is formulary-indicated for asthma maintenance and prophylaxis; it is added as a regular preventer when airway hyperreactivity is frequent enough to need more than an as-needed reliever.

Cautions
  • Inhaled, not swallowed: the monograph gives this strictly as an oral inhalation dose.
  • Regular inhaled steroid is for frequent/recurrent symptoms and is added to, not a substitute for, the as-needed salbutamol reliever.
  • Rinse the mouth after each dose to reduce the risk of oral candidiasis; hoarseness can occur.
  • Severe breathlessness or a silent chest is an emergency needing salbutamol and urgent transfer, not the preventer dose.
  • Frequent or worsening symptoms despite this dose should prompt formal asthma assessment rather than further self-titration.
