METOCLOPRAMIDE

Cautions
  • Extrapyramidal reactions can occur (acute dystonia, especially in young women) - limit to 5 days for this reason.
  • Avoid in Parkinson's disease and in known/suspected bowel obstruction or perforation.
  • Cardiac symptoms accompanying nausea need urgent cardiac workup, not just an antiemetic.
  • Severe dehydration or inability to keep fluids down needs IV rehydration, not oral therapy alone.
  • RED FLAG - Chest pain accompanying nausea, signs of raised intracranial pressure (severe headache, neurological deficit), signs of bowel obstruction or peritonitis, or severe dehydration or inability to keep fluids down, especially in pregnancy.

Cautions
  • Taken by mouth, not rectally: the dose text reads '10 mg orally up to three times daily'.
  • There is a QT-interval prolongation risk - avoid with other QT-prolonging drugs and in significant cardiac or electrolyte disease.
  • Does not cross the blood-brain barrier well, so lower extrapyramidal risk than metoclopramide - a reasonable alternative when EPS is a concern.
  • The maximum duration is one week per formulary; re-assess if symptoms persist.
