IS IT THIS? - reference only, to read alongside your own examination
  SYMPTOMS - what the patient reports (4)
    - Progressive deformity with painful erections marks the active phase, while a stable deformity
      for 3 months or more with pain resolved marks the chronic phase  [visible deformity]
    - Curvature exceeding 30 degrees is roughly the point at which it starts to interfere with
      intercourse
    - About 4% have a hereditary predisposition, and roughly 1 in 5 also have Dupuytren contractures
    - Patients usually seek care around 6 months after onset, most often for curvature, then pain,
      then a palpable plaque  [plaques]
  SIGNS - what you find (4)
    - Curvature of 20 degrees or less is essentially normal, while 60 degrees or more is usually
      unusable for intercourse and may need surgery
    - Self-reported curvature is unreliable; most patients overestimate it and only about 1 in 5 are
      close to accurate
    - The plaque is usually located on the top surface of the penis, and an hourglass shape or
      buckling under pressure during erection may be seen  [joint instability · plaques]
    - Larger calcified plaques over 1.5 cm predict a higher chance of eventually needing surgery
      [plaques]
  TESTS (4)
    - No lab testing or imaging is mandatory, and testosterone is only checked if hypogonadism is
      suspected
    - Major urology guidelines recommend an in-office injection test with or without Doppler
      ultrasound before any invasive treatment
    - Duplex ultrasound can find calcifications, assess blood flow, and detect plaques that are hard
      to feel by hand
    - MRI is not currently recommended for routine use because of its high cost and limited
      availability

    - A lifelong, gentle ventral curve with no plaque points to a congenital cause rather than
      acquired dorsal Peyronie disease
    - Ventral curvature accompanying hypospadias suggests chordee rather than acquired Peyronie
      disease
    - A sudden pop, immediate severe pain, and loss of erection during intercourse point to a penile
      fracture, not Peyronie disease
    - Pain without any deformity, possibly referred from the bladder, prostate, or back, points to
      penile pain syndrome instead

1. IBUPROFEN                                              [1st line]
   Adult    200 to 400 mg up to three times a day as required, with or after food. Leave at least
            four hours between doses and never exceed 1200 mg in any 24 hours.
   Peds     Adult-only condition - paediatric section not applicable

   Why      Ibuprofen is an NSAID used to relieve penile pain during the active inflammatory phase
            of Peyronie's disease; it controls symptoms only and does not reverse the fibrous
            plaque, which needs urology assessment.
   Caution  Oral NSAIDs aim to relieve pain in the active inflammatory phase; they do not reverse
            established fibrous plaque curvature.
            Urgent urology referral is required for assessment of intralesional injections or
            surgical reconstruction.
            Take with food to minimize gastric distress.
