Once infective and metabolic causes are excluded, urinary frequency/urgency reflects overactive bladder, for which solifenacin at the standard licensed starting dose is first-line.
- New excessive urination with thirst and weight loss suggests undiagnosed diabetes; frequency with blood in the urine needs further urological work-up.
- Only after excluding urinary tract infection and new-onset diabetes (both are red flags for this symptom) - antimuscarinics treat overactive bladder, not infection or hyperglycaemia.
- CONTRAINDICATED in narrow-angle glaucoma, severe ulcerative colitis, and urinary retention.
- Anticholinergic effects include dry mouth, constipation, and blurred vision; use cautiously in older patients because of the risk of confusion and falls.
